Why Pilates works for back pain - the BASI approach
Samantha Wood, who has integrated Pilates into physical therapy practice for over two decades, describes the core principle clearly:
"By strengthening the deepest muscles of the core, optimising alignment, and creating correct movement patterns, we can also help to prevent reaggravation of injuries and the development of new ones."
- Samantha Wood, BASI Faculty, Pilates for Injury Rehabilitation
The BASI method applies this through a structured, individualised approach:
- Exercises are chosen and sequenced based on the specific needs of the client, not a generic programme
- The goal is achieving optimal posture, functional strength, and balance - and rehabilitating the underlying injury
- The full BASI apparatus - Reformer, Cadillac, Wunda Chair, and Ladder Barrel - allows precise adaptation of resistance, position, and range of motion for every condition and stage of recovery
Pilates for Back Pain and Injury Recovery in Sofia
Back pain and injury change the way you move - often long after the initial cause has resolved. At The Black Swan Pilates in Sofia, we work with clients managing chronic back pain, lumbar dysfunction, postural imbalances, herniated discs, and post-surgical recovery. Our approach is grounded in the BASI method, founded by Rael Isacowitz, whose work emphasises that Pilates is not simply exercise - it is a holistic approach to optimising human movement.
Our lead instructor Elitsa holds a BASI Pilates for Injuries and Pathologies Certificate - the specialist qualification created and taught by physiotherapist and BASI associate faculty member Samantha Wood, MPT. This is the same course that underpins Pilates for Rehabilitation, the authoritative clinical guide recommended by BASI Pilates and used by rehabilitation professionals worldwide.
As Rael Isacowitz writes in Pilates (Human Kinetics, 3rd Edition):
"Pilates is a holistic approach to well-being that offers a bridge between mind and body, between everyday life and optimal performance, and between rehabilitation and healthy movement. When used to its full potential, it can enhance every aspect of life." - Rael Isacowitz, Founder of BASI Pilates
What to expect at The Black Swan Pilates
Every client with back pain or an injury begins with an Intro Private Session. This is not a standard first class - it is a full movement assessment. We review your history, understand your current limitations and pain patterns, and build your programme from what we find.
Sessions take place in our private studio at 23A Gorski Patnik St, Lozenets, Sofia - one instructor, one client, complete focus. Our team also includes Teodora, a qualified kinesiotherapist who graduated from the National Sports Academy Vasil Levski in 2023, bringing clinical movement knowledge directly into her Pilates sessions.
Conditions we work with
- Chronic lower back pain
- Lumbar disc pathology
- Postural imbalances and scoliosis
- Shoulder, hip and knee injuries
- Neck pain and cervical spine issues
Can I do pilates if I have a herniated disc?
In most cases, yes - with appropriate modifications. Samantha Wood's Pilates for Rehabilitation addresses lumbar spine pathology including disc issues specifically, with clear indications and contraindications for each exercise. We follow this clinical framework. Your Intro Private Session allows us to assess what is appropriate for your specific situation.
How is this different from physiotherapy?
Pilates and physiotherapy address different parts of the recovery process. As Samantha Wood writes, Pilates is particularly effective for neuromuscular re-education, functional strength, and long-term movement quality. Our team includes a qualified kinesiotherapist, and we work in close alignment with the principles in Pilates for Rehabilitation.
How many sessions before I feel a difference?
This depends entirely on your condition and history. Most clients notice improved movement quality and reduced discomfort within four to six sessions. Sustainable change in chronic conditions typically requires consistent practice over weeks and months - not a single intervention.
Is this suitable for me if I have had surgery?
Post-surgical clients are welcome, subject to medical clearance and an honest assessment of your current state. We always begin with a thorough first session before recommending any programme.
Do you work with clients who are also seeing a physio or doctor?
Yes. We encourage it. The BASI approach is designed to complement, not replace, medical care. If you are currently working with a physiotherapist, sharing their notes with us helps us align your sessions accordingly.
Your first session is a conversation as much as a workout. Tell us about your back, your history, and your goals - we build from there.
